When should back pain or bone pain be assessed for a stress fracture?
Children & adolescents
Children and adolescents can get occasional aches and pains, but persistent back pain or focal bone pain should not be ignored. In young athletes, pain that worsens with running, jumping, bowling, kicking, arching, or impact can be a sign of bone stress injury — including conditions such as lumbar spondylolysis (pars defect), tibial stress injuries or other lower-limb stress fractures.
Early assessment is important because accurate diagnosis and appropriate load management can reduce time away from sport, support healthy bone development, and lower the risk of recurrence. Back pain or bone pain that is persistent, recurrent, worsening, associated with night pain, causing limping, or limiting sport should be assessed by a clinician experienced in paediatric and adolescent sports injuries.
Adults
Adults can also develop stress fractures and bone stress injuries — particularly when training load increases quickly, recovery is reduced, or there are changes in running, gym, work, or sport demands. Persistent, localised bone pain that worsens with impact, loading, or repeated activity should not simply be managed with rest alone, as symptoms may settle temporarily but return when activity increases again.
A structured assessment can help identify the underlying cause, guide appropriate imaging (such as MRI or bone scan) where required, and create a safe, progressive plan back to activity, work, exercise, or sport.
